transgendered (TG)
Probably the umbrella term to mean any person who consciously crosses gender barriers on more than a casual basis. It is more of a state of mind than a medical condition. For instance, a guy going in drag while seeing the Rocky Horror Picture Show or dressing up as a woman on Halloween is proabably not TG. However, someone who routinely crossdresses or strongly internally identifies with the opposite gender probably is TG.
      Most TG people are not gay and not all act effeminate/butch. In fact, there are a lot of closetted TGs and you wouldn't even know it. Transsexuals are one segment of the vast TG community. Keep in mind that being transgendered is not directly linked to sexual orientation!
      Note: While I usually think of TG as including transsexuals, there is another perspective in which TG and TS are separate things. In this interpretation "transgendered" means a person who is of one genetic sex who believes themselves to be actually the other. But, "transsexual" in this context means that a person is actually of the other sex but born with a "birth defect" which makes them appear to not be physically that sex. (Confused?) This is a valid interpretation if you think of a MTF who has always identified as a woman yet was born with the birth defect of having a male body.
transsexual (TS)
An individual who is in the medical and legal process of becoming the opposite sex or who has completed the process. (Some people also apply "transsexual" to people who are very very TG but who choose not to take medical or legal action. I'm not going to use that definition.) The first thing that you probably think of is Jerry Springer with big guys in drag with a falesetto voice. While there are TSs who are like this, I have to say that most of the ones I have met personally are nice, intelligent, and pretty normal people. In fact, you may not even know that you're speaking to a TS.
      TSs are both those who are transitioning from male-to-female (MTF) and female-to-male (FTM). While MTFs are probably more prominent because of their physical characteristics, there many FTMs. Some people believe that the numbers of FTMs and MTFs are about equal. After transition, FTMs tend to disappear because a good many become undetectable. I once saw something about an FTM convention where an attendee remarked that FTMs often can't read other FTMs!
      A good number claim to have known from their earliest of their years that something wasn't right and it was not until later in life that they made the commitment to change sexes. Some grow up pretty normally and crossdress occassionally and only later figure out that they want to change sex. Many TSs don't get serious about their transition until their 40s---perhaps due to the so-called midlife crisis. Many internalize their anger and end up fighting against themselves. Some people find ways to not let their pain build up (i.e. me) but eventually it catches up to them. Not all TSs are suicidal. Some TSs are gay; some are not---see sexual orientation.
      The transition process is mentally, physically, and financially taxing. However, almost everyone that I've spoken to has had no regrets afterwards. (See transitioning below.)
      Some people use transsexual interchangably with transvestite and crossdresser, but there is a distinction. One way to look at this is to think of the transsexual as a transgendered individual who has actually begun (or completed) a physical and legal transformation from one sex to the other.
tranny
A transsexual. I guess you could use "tranny" to mean "transgendered", but for some reason it seems more fitting to me that tranny refers to people who alter their physical appearance.
MTF
Male-to-female transsexual. A person who was born male who becomes a woman. The individual does this by:

FTM
Female-to-male transsexual. A person who was born female who becomes a man. A person accomplishes this by:

intersex, intersexed
The condition of a person being born with ambiguous genitals. This rare condition makes genetic males appear to have female-like genitals and genetic females to appear to have male-like genitals. The true cause is not known but it seems to stem from hormone imbalances and/or chromosomal anomalties like having XXY genes instead of XX or XY. Children born intersexed are often taken in for various surgeries to make them seem to be normal men and women.
      Mistakes have been made and intersexed men have turned out to be more feminine after all, and vice versa. In the defense of the parents and medical staff, they considered that assigning the intersexed to be one sex or the other was in the child's best interest in order to not make them stick out as freaks in society. However, what they don't consider are the child's feelings---then again, the child is too young to really articulate what they want.
      Maybe the best thing is to create an environment where the child understands the he/she is not exactly normal but doesn't have to make up their mind until later? This could lead to embarassment for both the child and its parents. One suggestion I've heard is for the parents to initially decide which way to raise the child but not to have any surgery or hormone treatments until the child can decide...
queer
Usually a homosexual but also any non-straight or gender-bender person. Queer typically talks about a person's sexual orientation, but when it comes to the transgendered queer refers to anyone who has any kind of serious cross-gender identification or presentation. In a nutshell, if you're not a "normal" man or woman in your society then you are queer.
genetic girl (GG)
A person born female. Just a colloquial shortcut to refer to a person as a natal female versus a genetic male who transitioned to become female.
crossdresser (CD)
One who dresses in clothes almost explicitly associated with the opposite sex. The image this conjures up most often is a man in a dress with big hair, gaudy jewelry, overdone lipstick, and tottering on heels. That's probably the greater extreme---see "transvestite". Some men wear pantyhose, women's underwear, or some casual women's tops, so it's rather discreet. Of course, there's quite a few women who CD by maybe wearing boxers, cutting their hair short boy-style, or using men's wallets. Since our society sort of allows that (and some people even think it's cute) more women can get away with it. There's also people who run the gamut in between.
      Sexually speaking, probably more men get some kind of erotic satisfaction from CDing than women who CD. In the case of men, it may be because designers of women's fashion promote sex and there is a kind of "forbidden fruit" air about it. Face it: a man's suit doesn't really scream "sex" all over it. But what do I know? Maybe there are women who get aroused wearing men's dress shirts.
      It should be noted that not all men get aroused by wearing women's clothing. There are a number who CD just because it makes them feel better.
      Some people use the term transvestite interchangably with crossdressing, though I usually make a distiction. (See below.)
transvestite (TV)
One who crossdresses but usually with the intent to be showy, guady, or sexual. When someone says "transvestite", the images that come to mind are ballroom gowns, lots of glitter, and more expressive makeup. Whereas crossdressers may choose to do their crossdressing subtly, TVs are much more open about it. Camp humor and gay beauty pageants are good examples of it. The point of looking feminine in this case is more to draw attention than for personal pleasure. There may be a sexual element in the dressing as well.
female illusionists
A person who performs as a transvestite as their job. Female illusionists are mostly entertainers---so it doesn't necessarily mean that a female illusionist is transgendered though I'm betting most are. Want good examples of professional female illusionists? Try Asia SF, Bobby York, and Jim Bailey (who even appeared on Ally McBeal).
shemale
A person who appears to have a female body yet male genitals. Ah, one of the great mysteries of the transgendered. Typically appearing in pornographic and fetish websites and magazines, shemales usually have voluptuous bodies ... and a penis. Some of these erotic beauties could be the real thing (i.e. a genetic male who has taken hormones and undergone various feminization surgeries), but I'm betting most are pictures of GGs with male anatomies superimposed through photographic and digital editing techniques. In my opinion, shemales are not TS but closer to TV. Or maybe they are pure fiction created for the pleasure of those seeking alternative sexual fantasies. (Shemales are also known as "chix with dix" and "ladyboys".)
closetted
When a person keeps his/her sexual/gender preferences to him/herself for fear of what other people may think or how they may react. Talk about anxiety! Being closetted usually is a psychological burden because the person affected is afraid of letting hints about their personal problems become public knowledge.
      Sometimes this is a good type of paranoia if you happen to live in an environment which might harm you should you announce yourself. (For example, if you're a gay pro football player or a lesbian actress.) People lose their jobs, family, relationships, and sometimes their lives if knowledge of their private lives leaks out.
      People can remain closetted indefinitely and you may never know at all. In fact, the closetted person may even be closetted to him/herself---see denial. Regardless, the endless secrecy is hardly beneficial.
      A sort of weird closetting happens when a TS transitions and then goes stealth. Quite a few people say that you're just trading one closet for another.
non-genetic girl (NGG)
A girl who has always been a girl at heart but once had the body of a guy. In other words, an MTF TS who views themselves as a true female. (My friend Steven deserves credit for this term since he coined it. We throw this term around a lot at home.)
out, outted
The state of not being closetted any more. Some people are very openly "out" and not only have no qualms about their self expression but also make it hard to avoid not noticing. The other extreme is that a person may be out but never really say anything about it---you would have to ask to get any sort of information.
sex
A person's biological determination to be male or female. Basically, penis genitals = XX chromosomes = male, vagina genitals = XY chromosomes = female. In the perfect world, a person is either male or female, however Mother Nature throws a little variety into the mix and there are intersexed people or even XX females with an androgen insensitivity condition.
      A person does not have a choice in this matter as to what sex they are because genetics determine this. However, in the case of the TS, the person comes to the realization that their brain does not match their body and they seek medical help to change their physical body to match their desired sex. That is, their gender is not aligned with their sex.
      Some people hold the opinion that the brain is sort of hardwired to be male or female though the rest of their bodies is otherwise. There is mounting evidence that this may in fact be true due to research comparing the BTSc regions of the brains in biological men and women. (Frank Kruijver and colleagues published a study in The Journal of Clinical Endocrinology and Metabolism in the Netherlands.) Neuron counts have been performed on deceased transsexual men and women and compared with "normal" men and women---the FTM men had counts closer to normal men, the MTF women had counts closer to normal women. In an article by Robert Sapolsky, Sex-identity myths dispelled (USA Today), he writes:

Your pattern of chromosomes, gonads, genitals, secondary sexual characteristics, the hormones in your bloodstream and the way you are treated by your parents, teachers and society at large may all be agreement that you are of a certain sex. But something as hard-nosed and biological as the number of neurons in a part of your brain may be telling your mind that no, that's not who you are: You are the opposite sex.
Perhaps continued research will be able to shed more light on this subject.
gender
A person's internal and perceived identity of masculinity and femininity. For most people this is really clear cut: men are masculine and women are feminine. Even tomboys are feminine, effeminate men are masculine. When you look into a person and try to understand how they feel then you get closer to their gender. It's a personal thing and for the transgendered person their gender is fluid or even the opposite of their biological sex.
sexual orientation
A person's preference of men or women as sexual partners. Most people break this down into four categories:


One of the popular statements from the transgendered is that sexual orientation and gender are separate. Again, for the "normal" public this will seem ridiculous. But consider this:

heterosexual
The sexual preference of a person to want a person of the opposite sex as their partner. This sounds like a "duh" definition until you throw in transgendered persons. If a FTM has had top surgery and is living as a man (and may even have bottom surgery) and then he wants to find a woman, then is it a heterosexual relationship? Technically he has male parts and she has female parts now so it is heterosexual. But because he used to be a she then is it homosexual because he is a biological female? Hm... Technicalities like this are part of the reason why trannies may want to separate sexual orientation from gender.
homosexual, gay, lesbian
The sexual preference of a person to want a person of the same sex as their partner. Most people just use "gay" to mean homosexual these days. But if you want to categorize people then gay people are males who prefer males, and lesbians are females who prefer females.
queen
A homosexual male, perhaps somewhat flamboyant. Queens usually make it known by mannerisms or by just stating it.
swishy
That effiminate quality so stereotypically associated with gays. But, it's true. There is just something about the voice lilt, the candor, the gaze, the walk, the hand gestures that is neither masculine nor "mainstream" feminine. It has an almost operatic quality to it that's hard to concretely describe yet you know it when you see it.
bisexual, bi
A person who has no preference what sex their partner is. I guess you could interpret this two ways: the person likes sex in it various forms, or the person really just doesn't care what sex their partner is.
public display(s) of affection, PDA
Any physical act done in public which expresses the connection between individuals in a relationship. You know: hugging, kissing, holding hands, patting, petting, teasing, etc. The kind of stuff you sometimes see and think to yourself "hey, get a room!"
gender identity
The physical sex an individual identifies as. Most genetic males have a male gender identity, likewise most genetic females have a female gender identity. With the transgendered, the sex that the person associates with may be their own, the opposite sex, both, or none. In the case of the FTM and MTF they pretty clearly identify with the opposite sex otherwise they would not transition.
Gender Identity Disorder, Gender Identity Dysphoria (GID)
A person who strongly feels their internal gender identity does not match their physical sex. Sometimes GID-afflicted people decide to seek help and eventually transition to the opposite sex. Hence transsexuals. Some note that "Gender Identity Disorder" is more appropriate than "Dysphoria".
transition
The process of one changing their physical sex and legal identification. Some people draw a hard line and say that transition begins with the Real Life Test when they go full-time. Others start counting when they start coming out. Transition never really ends but 99% of it usually ends when the person has reached legal and/or surgical completion so that they live in their desired gender role. Some people never complete their transition and revert to their original gender. Some go part way through their transition and reach a point where they are comfortable and so they stop there, thus ending their transition. You can probably sum it all up as a person changing their physical appearance and then reaching a stage where they feel happy.
      Typical things people do in transition include:

Some say that FTMs have an easier time in transition due to the way hormones take effect. See HRT below.
Hormone Replacement Therapy (HRT), Hormone Therapy (HT)
Changing a person's hormone balance through medication. In MTFs they are given androgen blockers and estrogen. In FTMs they are given testosterone. The net effect of this causes the body to respond in a more feminine or masculine way. For example, MTFs develop breast tissue and become generally weaker. FTMs gain muscle mass and begin to grow facial hair (and even may acquire male pattern baldness). There are far more reaching effects emotionally and psychologically that take place.
      Some say that FTMs have it a little easier when it comes to HRT because of the nature of hormones. With a guy-ish haircut and strapping down their breasts, genetic females can usually approach androgeny. When testosterone is administered, not long after then their voices begin deepening and facial hair becomes apparent. With these audio and visual cues most people would assume these females to be men, albeit maybe a little bit smaller than the average guy.
      On the other hand, MTFs do not experience these kind of transformations as FTMs. About the only real things that hormones physically do to the genetic male are breast development and a little fat redistribution. (While there are many other changes, they aren't quite as obvious.) To help the genetic male pass as a woman the MTF must resort to surgeries and voice training.
      It should probably be noted that HRT is not just for the transgendered. Many GGs are seeking HRT as they reach menopause to prevent things like osteoporosis and hot flashes.
Breast Augmentation Surgery (BAS, BA)
Surgically enhancing your breast with implanted prosthetics. The boob job. The surgeon generally creates an incision either around the areola or in the muscle fold under the breast where a small inflatable pouch is placed. Then a filler (typically saline) is pumped into the pouches and that gives the user the enhanced breast size. The pouches are not lifetime; they expire after 10-15 years and need additional surgery. But for many people it is worth the pain and expense. Sometimes nerve damage occurs.
en femme
When a person appears/attempts to look, act, and speak as a woman.
en homme
When a person appears/attempts to look, act, and speak as a man.
clocked, read
When a person has been (or feels they have been) identified as being an impostor of the gender they are portraying. For a FTM it means someone figured out they were really a girl, for a FTM someone knows they are a guy. Being clocked is something trannies try to avoid and we have these mini celebrations when we pass.
pass, passing
The state where a male is treated as a female because he looks/sounds like a woman, or vice versa for females. This is the goal of many transgendered and is hard to achieve. One way of putting it is that if you saw a particular man on the street you would not know that he had been born a female. Not everyone can pass because of their physical size or features. (Hormones help a lot but don't alter the skeleton.) MTFs who have difficulty passing are usually larger or who have very deep voices. FTMs who have difficulty are usually short and come off as teenage boys.
fishy, fish drag
A MTF who passes very well. It is said that "fishy" people look really good, not just like a woman. With "fish drag" a person might look a little more glamorous, but still passes as a GG.
drag king
A female posing as a man. Typically campy, but sometimes stylish. (I don't have a good example, sorry.)
drag queen
A male posing as a woman. Typically campy, but not necessarily so. Think Ru Paul, Dame Edna.
mullet
A haircut worn short in the front and long in the back typically by males. Once popularized in the 80s it still continues to exist to this day as a visual reminder of the days of legwarmers and energetic teenie-bop music. Today you can often witness real live "mulleteers" in country music videos or NASCAR events. This has no relation to transgender topics but it appears here because the term "mullet" always brought a smile to my face. Consider this an "Easter egg".
androgynous, androgene, androgeny
When a person is neither perceived to be male nor female. When you meet a completely androgynous person, usually you have to wait for some kind of clue for you to figure out if they are man or a woman---e.g. hand gestures, voice, stance, conversational topics.
tucking
The "art" of hiding one's male anatomy. Basically you want something to be able to make the groin area appear as flat as possible. A combination of cutoff control-top pantyhose and maxi pads seem to work fairly well. Or doubling up on the underwear sometimes works. There are more sophisticated devices available.

If you've been on HRT for a while you may notice that tucking actually gets easier over time. There may come a time when the size of the testicles is small enough to be stowed away up in the inguinal canals.
part-time
A person who lives as their desired gender only some of the time. Most part-timers work or go to school as usual and change to their desired gender presentation once they get home. Some people change only for the weekends.

A part-timer everywhere but work.

full-time
A person who lives all the time as their desired gender. This means the person changes their legal identity, name, voice, and dress to match their target gender. There is no reverting back to their birth sex. A person usually is required to live full-time during their RLT so that they can see if they really want to continue with their transition. Think of it as "try before you buy". However, I think most people that reach this stage have undergone minor surgeries (e.g. facial feminization surgery, double mastectomy) and have started their HRT, so they have experienced some changes to their bodies which cannot be reversed easily.
real lift test (RLT)
An extended period of time that a person is required to be full-time. The RLT is usually a year or more and you usually have to do this before getting SRS. This is to force people to face the reality of waking up every morning in their target gender and being able to function in society. What you don't want to have happen is a person that cannot socialize outside of the transgendered community.
stealth
When a person is dressed as the opposite sex and they do not get clocked. You might say that this is the goal of most transsexuals. When you pass well enough to not be clocked then you can get on with your life.
sexual reassignment surgery (SRS), gender reassignment surgery (GRS) (penile inversion, sigmoid colon resection, phalloplasty, metoidioplasty)
The modification of genitals from the person's given sex to that of the opposite sex. In the MTF this is usually the construction of a vagina from the penis. For the FTM this is the construction of a penis in the area of the vagina---also referred to as "bottom surgery". You might also consider the double mastectomy also SRS for the FTM---"top surgery". These operations usually require the individual to have lived full-time for at least a year and have a letter of recommendation from their therapist.
      In the construction of the vagina for the MTF, a popular technique is penile inversion. The erectile tissues of the penis and the testicles are discarded. The urethra is shortened and sometimes even preserved for a later graft. The nerves of the penis are left intact and the skin is joined with the scrotal skin and inverted to create the vagina. After the neovagina has been inserted through the abdominal wall the clitoral hood is fashioned. Some surgeons do the clitoral hooding as a second operation. Costs vary though they seem to be now in the USD$12,000 to USD$20,000 range. After surgery the individual must dilate with stents to stretch the neovagina. A variation of this technique using a section of the lower intestine, the sigmoid colon resection, allows for nearly any depth however the costs are quite a bit higher.
      In the construction of the penis for the FTM, the techniques are quite a bit more invasive and expensive. Costs can range from USD$20,000 to USD$90,000. Popular techniques include the clitoral release (metoidioplasty) and phalloplasty (radial forearm free flap phalloplasty). In the phalloplasty typically a flap is taken from the forearm and wrapped around a catheter. With microsurgery the penis is reconnected in the groin area. While the individual can urinate through it and have sensation, erection isn't possible unless stiffening rods or some kind of a inflation system is used. With a clitoral release the clitoris (enlarged from HRT) is refashioned into a the glans on a small (yet functional) penis. (Good pictures are available in Loren Cameron's book, Body Alchemy.)
      A lot of people view "The Operation" as a sort of capstone of their journey. But it should be noted that not everyone has surgery. For quite a few transsexuals this is unnecessary. When you think about it, almost no one will see your genitals unless you need to strip down to your birthday suit. What really makes the difference in your life will be your emotional outlook and HRT.
stent
A medical device used to enlargen the neo-vaginal cavity. (Of course, this definition relates to SRS aftercare; medical dictionaries list stents as tube or balloon devices usually used to keep arteries open.) Usually made of highly polished plastic, stents are used in the process of dilation once the packing is removed from the neo-vagina. The user inserts the dilator below the pubic bone and holds it in place for several minutes applying steady pressure. Over time this should maintain and/or increase the size of the cavity. Water-based lubricants are recommended.
antiandrogen (medication)
Medication that blocks the effects of testosterone. By eliminating testosterone or diminishing its potency you allow the body to stop become more masculine. When using these drugs you may notice weight gain, loss of sex drive (almost certainly), loss of sexual function, muscle tone deterioration, and mood swings. You may experience slight breast growth or elargement. Some are diuretics which can be mildly annoying---drink lots of water, eat more salt, and sometimes you have to avoid foods/drinks high in potassium. Antiandrogens do not affect your levels of estrogen in the body already present. I look at antiandrogens as a way to buy yourself more time.
top surgery
Creating a male chest for a FTM. Involves more than removal of the breast tissue, the pectoral region is scultped into the male version.
bottom surgery
Creating the phallus and scrotum for a FTM. As described above in SRS, the groin region is reconstructed to create a penis which is functional and sensate.
facial feminization surgery (FFS)
Facial reconstruction of a more feminine appearance on a masculine face. The shape of the male skull and tissues are different than that of GGs. Various surgeries are available to advance the hairline, reduce the eyebrow ridge, enhance the cheekbones, recontour the nose, make the lips fuller, and reshape the jaw. (Some people also include the tracheal shave in this as well.) The obvious desired result of FFS is to allow the genetic male to pass as a woman. It should be noted that while hormones tend to help make the male face softer and fuller, it cannot restructure bone formation.
tracheal shave, Thyroid Cartilage Reduction (TCR)
Reduction of the male Adam's apple. The majority of women do not have an Adam's apple which is a trademark of typical male development. As a result most MTF TSs opt to get a tracheal shave. The cartilege prominence on the throat is reduced and the scars are usually hidden by the commonplace horizontal creases.
non-op
A person who has transitioned but chooses not to have SRS. Surgery is not everyone! Many people do what steps that they feel necessary to achieve a comfortable level of passability then then they stop.
pre-op
A person who is in the process of transitioning but has not had SRS yet. This is typical of full-timers who are in their RLT.
post-op
A person who has had SRS. A good number of people finally get the "operation" after they have lived in their desired gender full-time for an extended time, usually after RLT.
tranny chaser
A person who pursues transgendered people for sexual relationships. Somewhere lying between a sexual preference and a fetish, tranny chasers prefer partners who are in some way transgendered.
electrolysis
The procedure of permanently removing hair using a probe and controlled amounts of electricity. In short, you burn away the small clusters of cells that grow hair so that hair will never grow again from that follicle.
      The probe looks like a dull needle and is inserted into the hair follicles right alongside the hair shaft. Once under the skin electricity (alternating and/or direct current) is used to create a strong source of heat which literally burns the matrix cells at the base of the hair shaft. The idea is that after you burn these cells enough times that they will not be able to regenerate leaving the patient with no hair.
      Unfortunately the costs of doing this are quite a bit and it smarts like nothing else.
